Our Goal

The primary goal of LifeLink Hospice & Palliative Care is to be the provider of choice for Palliative and Hospice services. We aim to provide care and services to patients, caregivers, and families necessary for the palliation and management of the terminal illness and related conditions.

The Hospice Team

At LifeLink Hospice in Los Angeles County, our interdisciplinary team of skilled medical professionals delivers exceptional hospice care tailored to enhance the quality of life for patients and their families. With decades of combined experience, we address every aspect of well-being—physical, emotional, social, and spiritual. Our diverse team, including physicians, nurses, social workers, chaplains, and more, collaborates biweekly to review each patient’s needs, sharing specialized insights to create personalized care plans. This holistic, compassionate approach ensures we meet the unique needs of every individual and their loved ones with expertise and heart. Our team includes:

Hospice Physicians: Board-certified in hospice and palliative medicine, our physicians specialize in pain and symptom management for life-limiting illnesses. Available 24/7, they consult with families and ensure comfort at every stage of end-of-life care.

Nurses: Our registered nurses (RNs) and licensed vocational nurses (LVNs) coordinate care, manage symptoms, and provide hands-on support. From direct patient care to leadership roles, they’re complemented by after-hours nurses offering phone guidance or home visits as needed.

Hospice Aides: Certified Home Health Aides (CHHAs) assist with daily tasks like bathing, dressing, and light homemaking. They empower families with guidance on caregiving, ensuring patients remain comfortable and supported.

Social Workers: Our master’s-level medical social workers address emotional and practical needs, helping families navigate tough decisions. They facilitate discussions, secure resources, advocate for patients, and assist with placement needs.

Spiritual Coordinators: Our spiritual coordinators offer compassionate support for existential or spiritual concerns, tailored to each patient’s beliefs. Whether faith is central or not, they honor every individual’s journey without judgment.

Volunteers: Integral to our team, volunteers provide companionship, run errands, or offer caregivers brief respite. They also assist with office tasks, ensuring seamless care delivery. Their presence brings comfort and connection to patients and families.

Bereavement Support: Our care extends beyond a patient’s passing, offering grief support for at least one year. Families and the community benefit from counseling, phone check-ins, newsletters, workshops, community resources, and an annual memorial service to honor loved ones.

Special Services: We enhance well-being with unique therapies, including music therapy, pet therapy, Reiki, mindfulness meditation, massage therapy, and palliative sedation, providing comfort and emotional support for patients and families.
