Levels of Care

Continuous Care

Short-term crisis management is provided in the patient's place of residence. Most often initiated when death is imminent and the primary caregiver is unable to cope. Hospice staff will provide 24-hour care at this level.

In-Patient Care

Short-term crisis management when symptoms cannot be managed in the patient's own home. Patients have an option to have their needs met in one of our community hospice houses. Room and board are covered at this level and all hospice facilities are Medicare certified.

Routine Care

Hospice staff will provide regular, scheduled visits to a patient's place of residence to adequately manage symptoms and meet the needs of the patient and family.

Respite Care

Intended to provide short-term relief for the primary caregiver. The patient can stay at one of our hospice houses for a period of up to five days. Room and board are covered.

Who is Eligible?

Lifelink Hospice & Palliative Care serves individuals who have a life-limiting illness and those who choose comfort care. Our services include providing care for those who would like to focus on managing symptoms and controlling pain, while preserving their comfort and dignity.

We provide care where the patient stays, and can accommodate any setting. Services are covered 100% by Medicare, and most insurance plans. We will provide services regardless of the patient's ability to pay.
